¡¤¡¤SELF-PAY-ELDERS NOTICE ¡¤¡¤

Self-pay-elders must notice:
1. Every self-pay-elder should bring a copy of the registered permanent residence book, original ID card and an ID card copy. In addition, they should bring 5 1-inch recent color photos of both themselves and their warrantors.
2. Before living in the center, we will check the elders' health condition.
3. Before living in the center, every self-pay-elder should register, fill out an application, sign the Agreement Contract and Medical Treatment Position Paper, and then pay an attendance fee.
4. Every self-payer should pay the attendance fee on the 10th to 12th day of every month. There will be a late fee if the payment is made after the 12th.
5. The elder's name should be embroider on his (or her) clothing and requisites. His (or her) trousers' should have an elastic waistband instead of a belt and diapers for incontinent elders should be prepared.
6. The center will provide living necessities for the elder on the first day. If the elders damage something, such as the bed quilt or the mosquito net etc., the elders' relations should compensate the center.
7. Every elder must sign out before leaving. At that time, we will inform the elder's warrantor to insure the warrantor agrees with the elder's leaving. The warrantor should complete some associated procedures about the elder's leaving. We will return the meal fee to the elder during the time he (or she) is absent.
8. If the elder wants specific care, such as additional morning tea, refreshment, Chinese traditional medicine, more dishes and so on, they should pay the basic fee for the additional service.
9 After a consultation with their family numbers, we will change the attendance fee according to the different health situations of the elders.
10 In addition, we shall adjust the elders' bed according to the actual needs of elders.
